Job Description:
We are seeking a Case Management Registered Nurse to join our team! You will be responsible for monitoring and assessing the needs of patients, determining treatment eligibility, using clinical options to decide if plans meet the patient’s needs, and making arrangements for additional services that help patients achieve a positive outcome.
